<div><strong>Job Profile:</strong><br/><br/><ul><li>Are you approachable, always willing to help your team and keep your positive attitude? </li><li>Do you want a great relationship with your team that you're excited to be at work even on challenging days? </li><li>Do you desire to work independently at the top of your scope? </li><li>Do you want to collaborate with providers as an equal person? </li><li>Do you lead by example? </li><li>Are you bilingual in English and Spanish?</li></ul><br/><br/><strong>What You Get to Do:</strong> <br/><br/><ul><li>Provide remote nursing support to clinic teams by focusing on RN tasking, lab reports, patient communication, and care coordination. </li><li>Complete nurse lab reports in the morning and ensure timely review, follow-up, and documentation. </li><li>Manage the clinic task box and patient portal in a timely and accurate manner, including patient outreach, education, and coordination with providers and care teams. </li><li>You will support abnormal lab result workflows and treat per protocol when appropriate. </li><li>You may be asked to help with outreaching any walk-in patients requiring triage. </li><li>Collaborate with providers, pod teams, and remote nursing team to support and help facilitate patient care. </li><li>Occasionally assist remotely with other sites with high task volumes? </li><li>Although this role is primarily remote, team members may be asked to assist in person to support clinical coverage. <br/><ul><li>You will attend to the needs of the team including injections, medication administration and patient education. </li><li>You will get to work at the top of your scope doing independent Nurse Visits such as wound care, managing INRs, Hypertension visits, UTI's, etc. </li><li>You get to do Co-visits where you will evaluate patients, in collaboration with the provider, and treat patients with acute needs to help create access for patients. </li><li>Assess walk-in Triage patients </li><li>You will build teamwork by attending huddles before the start of shifts to review any anticipated nurse needs during that shift. </li></ul></li></ul><br/><br/><strong>Work Environment:</strong><br/><br/><ul><li>Remote workspace at home must include a HIPAA compliant space.</li><li>**Primary Location is subject to change at any time based on organizational needs** </li></ul><br/><br/><strong>Compensation</strong>: Approximately $34.70 - $43.95 per hour.
